TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Webster, New York, 14580 About The Position Job Title: Pediatric RN - Home Care CHHA/Oasis Job Specialty: Case Manager RN Job Duration: 13 weeks Shift: 3x8.0, Friday - Sunday, 8 AM - 4:30 PM Guaranteed Hours: 32 hours/week Experience: One year of experience as a clinical pediatric nurse preferred, home care experience preferred License: Valid State RN License Certifications: Basic Life Support (BLS) Must-Have: - Strong interviewing, assessment, and communication skills - Ability to exercise initiative and independent judgment - Ability to build and maintain professional relationships - Current driver's license - Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments Job Description: - Develop care plans appropriate to the patients' and families' health care needs in collaboration with patients, families, physicians, clinical operations team, and third-party payers, including obtaining medical orders and approval of medical services. - Complete the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient. - Ensure the completion of all appropriate clinical records needed for compliance with state and federal legislation and deliver seamless service within established time frames. - Refer to appropriate health care systems and community agencies/resources. - Communicate with health care team members about the outcome of the assessment/reassessment (if applicable) visit to ensure appropriate follow-up occurs based on the needs of the patient's plan of care. - Display an exemplary level of patience, courtesy, and flexibility and interact with patients, physicians, and staff in a manner conducive to maintain positive relationships and meet the agency's goals and objectives. - Ensure problem resolution at the point of contact. - Maintain appropriate productivity requirements. - Adhere to Professional Home Visit Performance Standards. 26232781EXPPLAT Job Requirements Required for Onboarding ACLS - AHA BLS - AHA Pharmacology Skills Checklist Specialty Exam TNCC About Cynet Health Your travel nursing adventure begins here.
