Job Description:

A company is looking for a Bilingual (English/Spanish) Cardiac Health Coach.

Key Responsibilities

Conduct initial assessments for patients enrolling in the virtual cardiac recovery program
Educate patients on program benefits and risk factor modification
Complete individualized treatment plans, including nutrition, psychosocial, and fitness assessments

Required Qualifications

A minimum of three years of experience in health management, preferably in a clinical or remote setting
A degree in a health-related field
Experience in counseling or education of disease management
Some knowledge of cardiac rehabilitation
Experience working with an elderly patient population
